Management of Various Tachyarrhythmias with Antiarrhythmic Drugs to Address Clinical Challenges
The “Workshop on Pharmacological Management of Tachyarrhythmias: Principles and Clinical Application” was held in the Dubai Room, with Sumarni, MD, PhD, serving as Course Director. The afternoon program opened with introductory remarks, followed by a pre-test for participants.
The scientific sessions began with Fera Hidayati, MD, who presented “Initial Approach to Tachyarrhythmias: When and Which Antiarrhythmic to Choose?” The session reviewed key principles of Advanced Cardiac Life Support (ACLS), emphasizing a structured approach based on ECG interpretation and hemodynamic assessment. Interactive questions throughout the presentation encouraged participants to apply the ACLS algorithm to clinical scenarios.
The pharmacological principles of antiarrhythmic therapy were then explored by Alice Inda Supit, MD, through “Differentiating Vaughan Williams Class I–IV: Practical Indications and Contraindications.” The presentation highlighted commonly used class IC agents, including propafenone and flecainide, and emphasized that these drugs are generally reserved for patients without significant structural heart disease and with preserved ventricular function.
Following an interactive discussion and coffee break, Fera Hidayati, MD, continued with “Antiarrhythmics in Atrial Fibrillation: Rate vs. Rhythm Control in Emergency Settings.” The session outlined the principles of rate and rhythm control, including clinical considerations that guide the selection and implementation of each strategy in acute settings.
The workshop then turned to ventricular arrhythmias with Alice Inda Supit, MD, presenting “Managing Stable Ventricular Tachycardia: Know Your Weapons.” The session reviewed the recognition and management of stable ventricular tachycardia, emphasizing its distinction from supraventricular rhythms with aberrant conduction and the importance of selecting appropriate therapeutic interventions.
The final session, “Case Simulation: Polypharmacy and Proarrhythmic Risk Avoiding Treatment Pitfalls,” was led by Sumarni, MD, PhD. Through case-based discussion, participants explored the narrow therapeutic window of antiarrhythmic drugs and the increased risk of adverse effects and proarrhythmia in older patients. A systematic approach to minimizing treatment-related risks was emphasized.
The workshop concluded with an interactive discussion and post-test, reinforcing the principles of safe and effective pharmacological management of tachyarrhythmias.
