Shaping the Future of Cardiac Electrophysiology: Innovations in Ablation, Pacing, and Arrhythmia Management

Exploring emerging technologies, physiologic pacing strategies, and evolving approaches to arrhythmia management

The scientific program started with Symposium 1: Live Demo Pulsed Field Ablation (PFA) which was moderated by Prof. Yoga Yuniadi, MD, PhD and Ignatius Yansen, MD. The session highlighted recent developments and practical applications of pulsed field ablation (PFA) in cardiac electrophysiology, particularly for atrial fibrillation (AF) treatment.The symposium began with De Novo AF Ablation: Strategies for First-Time Success, presented by Sunu Budhi Raharjo, MD, PhD. The presentation emphasized procedural planning, appropriate patient selection, and strategies to achieve successful outcomes during first-time AF ablation.This insightful presentation was followed by explanation about PFA Clinical Application Overview delivered by Prof. Liqang Ding, MD, PhD who discussed the clinical applications and potential advantages of PFA compared with conventional ablation techniques.The highlight was the last agenda, which is The Live Demo Pulsed Field Ablation performed by Seow Swee-Chong, MD and Budi Ario Tejo, MD from Fatmawati Hospital Indonesia, providing participants with practical insight into PFA procedures and critical core of the procedure.

Scientific programme was then continued by The Symposium 2 : Conduction System Pacing : A Paradygm Shift in Resynchronization?, moderated by Faris Basalamah, MD, PhD and Anggia Chairuddin Lubis, MD, explored recent advances in Conduction System Pacing (CSP) and its emerging role in cardiac resynchronization, particularly in patients with advanced heart failure. The first presentation, CSP, CRT, or Combined Approaches in Advanced HF Patients was delivered by Giky Karwiky, MD which focused on clinical considerations for selecting CSP, conventional Cardiac Resynchronization Therapy (CRT), or combined strategies. The following presentation was delivered by Koh Keng Tat, MD, who presented Bachmann’s Bundle Pacing: New Physiologic Pacing Insights for Atrial Resynchronization, insightful presentation which discussed its potential to optimize atrial activation and synchrony. The final presentation, Pace and Ablate Strategy Patients in Conduction System Pacing Era – Who Will Benefit? by Muhammad Yamin, MD, PhD added more insight to address patient selection and the rationale for pace-and-ablate strategies. 

Last agenda was completed with Symposium 4: Debate Session : Asymptomatic High-Burden VPC: Ablate or Not to Ablate? which moderated by Beny Hartono, MD, and Hermawan, MD. This session opened with the pro-ablation position was presented by Dicky Armein Hanafy, MD, PhD, who highlighted the potential role of early ablation in reducing VPC burden and preventing VPC-induced cardiomyopathy. Conversely, Prof. Muzakkir, MD, PhD advocated for conservative management, emphasizing individualized risk assessment, close monitoring, and the avoidance of unnecessary procedural risks. The subsequent rebuttal session provided a particularly insightful exchange with both speakers critically addressing the evidence and clinical implications of the opposing approach. Overall, the debate underscored that management should not rely solely on VPC burden, but should incorporate ventricular function, arrhythmic risk, patient characteristics, and long-term clinical trajectory.
